COVID-19 OFFice information

When Visiting our office you MAY be asked questions upon arrival such as:        

• Your recent travel history       
• Further detail of past medical history        
• Recent exposure to COVID-19

- Masks are OPTIONAL at our Office. If you feel you would like to wear a mask while in the office we encourage you to do so. If you request the staff to wear a mask while you visit the office please inform us prior to you appointment so we can accommodate your request. 

Things our office is doing to keep you safe:                 

• Additional cleaning and sanitation will be done throughout the day on a daily basis.
